Upon 30-day written notice of request for account cancellation, the following access and services will be unavailable on the date of service inactivation:

1. Provider will lose the ability to invite and/or order DROWZLE products for patients. All links and codes to register for associated programs will be inactivated.
2. Provider Portal access will be suspended and access to patient testing information and associated reports will no longer be available after account inactivation.
3. Patient access to all DROWZLE programs including previous recording results and reports will be also be suspended.
4. Requests must be made before the billing date to receive a refund. Requests made after the billing date will NOT be refunded. You will continue to have access to your account until it is deactivated.

It is the responsibility of the provider and associated users to export any key reports and user records prior to cancellation to retain historical information. Providers may wish to notify their DROWZLE product users of the termination of service to allow them to also download and/or print reports they wish to retain.
